• Picking you up at the facility and taking you home
• Clear communication of discharge orders to the patient and their loved ones
• Getting an appointment with your physician for tests or labs
• Coordinating delivery of durable medical equipment (DME)
• Faxing your new prescriptions and discharge orders to your primary physician
• Reminding you to pick up your prescriptions and setting up your Mediset
• Buying groceries and cleaning your refrigerator
• Help with light housekeeping, errands, and laundry
• Meal preparation that accounts for dietary restrictions
• Helping you set up your home to create a safe environment
• Help with dressing, bathing, and other daily activities
